Waiver and acknowledgements required for participation in the QCYC Sea Scouts sailing program

Waiver of Liability

I recognize that risk of injury or potential health risk, including contracting COVID-19, may be involved in participation in the above-named program/activity. I hereby willingly assume such risk of injury for myself or for the above-name person(s) for whom I am in law responsible and assume full responsibility before, during and after my/their participation in the program/activity and any associated or related activities. In consideration of the acceptance of my application and the permission to participate in the program/activity, I, for myself, my heirs, executors, administrators, successors and assigns HEREBY RELEASE, WAIVE, AND FOREVER DISCHARGE the Queen City Yacht Club, and all its members, directors, agents, employees, officials, servants, contractors, representatives, elected and appointed officials, successors and assigns OF AND FROM ALL claims, demands, damages, costs and actions whatsoever and however caused, arising or to arise by reason of my participation in the program or any of its associated activities. Further, I understand that students must wear a DOT approved lifejacket at all times as instructed, abide by the rules, including wearing a mask, maintaining physical distancing and washing hands, and co-operate with the instructors or risk expulsion. No refunds will be issued for students that are expulsed for failure to abide by these conditions. Further, I understand that the information QCYC collects on this form is used solely for the purpose of QCYC program registration and fulfillment. QCYC does not release this information to other parties and will only use the information as described herein.

Rowan’s Law

As per Rowan’s Law, it is mandatory to ensure that athletes, or parents/legal guardians of athletes under 18, confirm that they have read a Ministry Approved Concussion Awareness Resource, and they have reviewed, and committed to, the Ontario Sailing Athlete Concussion Code of Conduct.

Review the Ministry Approved Concussion Resources and the Ontario Sailing Concussion Codes of Conduct.

I (as participant or parent/legal guardian if the participants are under 18) confirm that I have reviewed the provided Concussion Resources and I acknowledge and commit to the Ontario Sailing Concussion Code of Conduct.

Safe Sport

The Queen City Yacht Club “QCYC”, in cooperation with Sail Canada and Ontario Sailing, is committed to providing all participants involved in sailing at QCYC, a safe and welcoming environment. Sail Canada and Ontario Sailing have created a Safe Sport policy package which sets the expectations for participation in sailing. QCYC has adopted the Safe Sport policy package and requires that all participants acknowledge and abide by QCYC’s Safe Sport policies.
