Sea Scouts Intro to Scuba – April 6 – 403rd Toronto Sea Scout Group

We need the following information and forms by March 19 please. You can submit the forms with the file upload field at the bottom of this page or you can email them to seascouts@403to.ca. The forms were emailed to you, but can also be downloaded with the links near the bottom of the page.

For the following measurements, be sure to include the units with the data you give, so we are not guessing. For example if you give a height in cm, please write it like so: 182cm. Or in Imperial: 5′ 11″. Same for weight: indicate whether it is in kg or lbs.

For shoe size, because North American sizing is just an arbitrary number, be very sure to indicate if the number is a men’s, women’s, or youth size.
